Certificate of Disability for Discharge.
John M. Shinn Private of Captain G.D. Stewarts Company K, of the Sixteenth Regiment of United States Volunteers, was enlisted by George D. Stewart, of the Sixteenth Regiment of Illinois Vol. at Quincy, Ill. on the Twenty Fourth day of May, 1861, to serve Three years; he was born in Summerhill in the State of Illinois, is Twenty Six years of age, 5 feet 8 inches high, Dark complexion, Hazel eyes, Dark hair, and by occupation when enlisted a Farmer. During the last two months said Soldier has been unfit for duty 60 days.
Every little exposure or change of climate affects him so much so as to render him unfit to perform the duties of a soldier.

I certify that I have carefully examined the said Private John M. Shinn of Captain Stewarts Company, and find him incapable of performing the duties of a soldier because of chronic diarrhea and general debility - contracted in the service though of a feeble constitution.

Widow's Claim for Army Pension.
State of Illinois, County of Pike, --ss.
On the eighteenth day of May A.D. one thousand eight hundred and Sixty-three, personally appeared before me, a clerk of the County Court within and for the County and State aforesaid, Mrs. Louisa Shinn a resident of Atlas Township in the County of Pike and State of Illinois, aged thirthy two years, who being first duly sworn according to law, doth on her oath make and following declaration, in order to obtain the benefits of the provisions mad by the Act of Congress, approved July 14, 1862:
That she is the widow of John M. Shinn, who was a Private in Company K, commanded by Captain George D. Stewart, in the 16th Regiment of Illinois Infantry Vols, in the war of 1861, who was honorably discharged on the 29th day of May ss 1862 by reason of chronic Diarrhea and general debility contracted while in said service and that on the next day after his arrival at his home in Atlas Township in the county and state aforesaid, he died from the effects of the diseases above named his said death taking place on the 14th day of June in the year of our Lord one thousand Eight hundred and Sixty two ss 1862.
She further declares that she was married to the said John M. Shinn on the 13th day of February in the year 1853; that her husband, the aforesaid John M. Shinn died on the day above mentioned, and that she has remained a wifow ever since that period, that she has five children by her marriage with the above named John M. Shinn. That they all reside with her at her residence as aforesaid that their names and ages aare as follows: Daniel D. Shinn aged nine years, Jesse J. Shinn aged six years, William P. Shinn aged five year, Merrick Shinn aged three years, Rachael Elizabeth Shinn aged one year.
As will more fully appear by reference to the proof hereto annexed. She also declares that she has not in any manner been engaged in, or aided or abetted the rebellion in the United States. And she hereby appoints Richard M. Atkinson of Pittsfield Pike County Illinois her attorney, with full powers of substitution in his won name and stead as fully and complete as she herself could do if personally present, to prosecute her claim and receive her pension certificate.
